1911 malfunction that can only be the unluckiest guy on the planet ......or possibly the luckiest ....... There's a probable answer 8-9 posts down but what are odds of a partial non-stove pipe ejection going back in the chamber head first ?

Replace the ejector and make sure your springs match your loads. I knew a guy who could make it happen every few mags; he said it was a combination of the ejector being slightly "off" and the loads barely working the action.

FWIW, his gun was tuned for softball loads; his trick load was a 185 swc at just under 700 fps.
